Registration Checklist for Parents

To Complete the Registration Process for Your Child, Please:

• Complete, Sign and Return the Registration for Enrollment Form (below)
  
• Submit the Registration Fee of $90 per child (maximum fee of $110 per family)
  
• Pay the last month's (June) tuition security deposit 
• Provide a copy of your child's birth certificate
• Complete, sign and return Permission Agreements, Health Policy, Financial Agreement, and Step By Step Preschool Enrollment (Emergency Contact Form) (below)
  
• Return completed and signed Child in Care Medical Statement (or comparable physician's form)
